How can we help?
One of the reasons we started Sophia's Promise was to help families with a newly
diagnosed child make the difficult life transition a little easier. You're not alone.
We'd love to send you a Care Package Kit that includes a Gift Card to Walgreens (or
substitute if outside of the United States) and a Personalized Medical ID Bracelet.
Absolutely FREE...there are no strings attached. All you have to do is e-mail the
following information to us at cpkit@sophiaspromise.org:
- Your name and mailing address so we can send the package to you
- Color preference (please provide a few) for your child's Medical ID Bracelet
- Information to include on your child's Medical ID Bracelet. It will read:
- Your child's first and last name
- Type 1 Diabetes
- Phone number to contact in case of emergency
